Required Documents

In order to begin services the following documentation are required:

  • Comprehensive Diagnostic Evaluations

    • Developmental Pediatricians

    • Neurologists

    • Psychologists

  • Order Request/Referral

    • ​Signed by an MD including ICD-10 diagnosis codes

  • Letter of Medical Necessity

  • Individualized Education Plan (IEP)

    • Required if services are provided in school
